Hospital is recruiting for EM physicians to work in our Emergency Department (ED). The ideal candidate should be EM trained and seeking full-time employment. We offer both hospital-employed and independent contractor positions.
While the current Medical Director continues to be an outstanding leader he is looking to transition an additional Director onto the team an eventually pass this responsibility along.
The ED has four clinical areas (Emergency, Fast Track, Emergency Decision Unit and Behavioral Decision Unit), and a dedicated area for the evaluation and care of patients who are victims of sexual assault. We serve the population of the “ North Country” and see over 50, 000 patients annually. A recent expansion, that doubled the size of the ER, opened in 2011. The EMR being used is SORIAN.
- The Emergency area treats emergent and urgent classifications of patients in a 27-treatment space unit (20, 000 Square feet). There are three trauma/cardiac bays and 24 multi-purpose rooms. The department also has a main waiting room, triage area and a family-consult room.
- The Fast Track consists of seven multipurpose rooms and a reception/waiting area. Fast Track hours of operation are 9am-11pm. It is staffed daily with PA’ s and NP’ s working two shifts (9am – 7pm and 11 am – 11 pm). There is one additional PA/NP shift in the main ED daily.
- The Sexual Assault Care Room is a specially designed room to address the particular needs of the victim of sexual assault.
- The Emergency Decision Unit has nine rooms with Central ED Cardiac Monitoring System and conduct Exercise Stress Tests.
- The Medical Center received a $3.6 million grant to expand the Emergency Department by adding an additional nine beds to alleviate patient congestion/hall patients.
- The ambulance bay was transformed into a four-room treatment area specifically designated for mental-health treatment.
- The Behavioral Decision Unit is a safe area consisting of four private rooms. Rooms have capability for continuous visual monitoring.
- The Clinical Decision Unit has six rooms with radio frequency capability to a Central ED Cardiac Monitoring System.
The department is set-up to support any type of patient in any room with equipment moved to the patient rather than vice versa. There is a general radiographic room with CR with PACS in the ED that is staffed 24-hours a day, and a 64-slice CT Unit for ED patients.
There are a total of 87 FTE’ s (non-physician) in the Department. All staff members are required to successfully complete Basic Life Support and Advance Cardiac Life Support Courses. Staff is also provided with the Emergency Nurse Pediatrics Course, Trauma Nurse Core Curriculum, and Pediatric Advanced Life Support training in-house. Clinical Assistants cover Stress Tests, EKGs, Phlebotomy, vital signs, turn and reposition work, etc. The Emergency staff is also responsible for staffing the Decision Unit. Staff members belong to one of two unions, depending on job title; the NY State Nurses Association or SEIU 1199.
Other support staff includes: Director, Assistant Director, Clinical Practice Coordinator (1), Clinical Coordinators (5).
EMT at facility is the hospital affiliated ambulance company. The service presently has 36 FTEs and 5 ambulances, and reports through the Manager of EMT Services to the Vice President of Planning and Professional Services. EMT’ s from this service work as clinical support staff in the ED when not on an ambulance run.
Shifts are flexible (8, 10 and 12 hour) and scheduling provides 60 hours of physician coverage. Providers typically see fewer than 2 patients per hour. We have 14 FTE physicians that staff the unit. The ED docs do not admit patients. The primary care physician or specialist admits them. Documentation is 100% in the EMR (Sorian 3.3).

Hospital is a private, non-profit, NYS designation level II trauma center serving a tri-county area of 170, 000 in northeastern New York. It is a regional tertiary center that offers high technology and is JCAHO accredited. It is one of eight state-designated Regional Resource Centers for Emergency Management. Of significance is that we are in the process of earning American College of Surgeons Verification as a Level III Trauma Center. It is recognized by Healthgrades as a Top 100 Hospital for Cardiac Care.
Our typical daily census is approximately 284 (approximately 170 acute beds) with 165 physicians on the Medical Staff. Our sub-specialty support is very good. In 2015 the hospital had 10, 220 admissions, 96, 360 patient days, 1, 095 births, 51, 100 emergency room visits and 22, 265 surgical procedures. We are one of the few New York hospitals that typically operate in the black. We are much bigger than what you would expect in a rural area.
The services offered here are advanced and our technology vast. Our state of the art critical care unit has 14-beds, the Progressive Care Unit has 12 beds and our three cath labs have digital equipment. We have newly constructed OR’ s (9), Emergency Department, main lobby and patient registration areas. The cardiovascular short-stay unit includes 9 rooms for post-procedure and cardiac access patients. Our Ambulatory Surgery Center has 4 OR’ s and 4 endoscopy suites. Our newest Medical Arts Building houses a state of the art laboratory, pharmacy, outpatient cardiology services, and physician office space. Exciting news on the horizon is that the NYS Department of Health has approved our plan to establish the Northern New York Center of Cardiac Excellence. The centerpiece of the program will be our angioplasty and EP services which has gained much recognition for its quality outcomes and increased volumes.
The hospital has made good progress on the EMR front. We utilize Soarian Clinicals for Computerized Provider Order Entry (CPOE) and MAK for Bar Code Medication Administration (BCMA). We have electronic results for imaging and lab with the ability to view actual images linked through Soarian. We also feed much data to HIXNY, the regional health information data warehouse that can be accessed from the provider’ s office, and in some cases we feed results directly into office EMRs. All inpatient units except Behavioral Health and the nursery are documenting nursing data in Sorian. Vital signs and I & O in the ICU and Progressive Care Unit are still on paper flow sheet. The ED is 100% paperless and we have plans to bring this system live to the other inpatient units in the medical center.
